If you deactivate your account, you lose access to your MyChart account. You are logged out automatically and can no longer log in. You also stop receiving communications directing you to log in to MyChart.

• If you have connected your account to any other organizations, you can no longer see information from your deactivated account.
• Other MyChart users who have access to your medical information keep that access even after you deactivate your account.
• Your medical information is not deleted. Your healthcare organization may keep your information on file to ensure you continue to receive the best possible care.

You can deactivate your account without being logged in. If you do not remember your username or password and want to deactivate your MyChart account, contact your healthcare organization's MyChart help desk. You can usually find the contact information on their FAQ page, which is linked on the bottom of their MyChart website.

You can sign up for MyChart again in the future. To reactivate your account, contact your healthcare organization.
